New Patients
Welcome! We’re glad that you’re calling SVT Health Center your health care home.
Before your first appointment, please take a moment to print and fill out the New Patient Forms.
What to bring to your first appointment:
- Your completed New Patient Forms
- Medical Insurance Card
- Medicaid Coupon
- Medicare Card
- Proof of Income if you would like to apply to receive a discount or are uninsured (pay stubs, tax return, bank statement, unemployment benefits statement)
- Photo Identification
- A list of all medications you are taking (or bring the medications with you)
- Immunization records if requesting an immunization
New Patients Please plan to arrive 15 minutes before your scheduled appointment or present 5 minutes early if all of your new patient forms are complete.
Current Patients
Please plan to arrive at the SVT Health Center at least 5 minutes before your scheduled appointment time. If you’re unable to make your appointment, kindly give us at least 24 hours notice.
If you are more than 5 minutes late for an appointment, we will consider the appointment missed and will reschedule for a later date.
SVT Beneficiaries
As a federally recognized community health center, SVTHC requires income verification once per year. This could be a pay stub, a tax return, a bank statement or a benefits statement, anything that can prove your annual income. This allows SVT to recover a percentage of federal government funds so that we can continue to offer high quality health coverage to our beneficiaries.
Every year, SVT publishes a Health Care Update to beneficiaries outlining the programs and services available.
